The Signs That Get Blamed on Everything Else
Fatigue is the headline symptom, but it's rarely the only one — and it's the one most likely to get written off as stress or bad sleep hygiene. Brain fog belongs on the list too: iron feeds the enzymes your brain uses to make dopamine and norepinephrine, so when levels drop, concentration and short-term memory are often the first things to slip. Pregnancy roughly doubles iron needs, since the body is building a placenta and an entirely new blood supply from scratch. Add a diet that's light on red meat, frequent blood donation, or a gut condition like celiac disease that blunts absorption, and the math stops working even for women who eat reasonably well. Men don't carry a built-in monthly deficit, which is a major reason iron-deficiency anemia gets diagnosed in women at several times the rate it's diagnosed in men.
Heme vs. Non-Heme: Not All Iron Behaves the Same
Heme iron comes from animal blood and muscle tissue — beef, lamb, dark-meat poultry, sardines, oysters — and the gut absorbs it efficiently, often 15 to 35 percent of what you eat. Non-heme iron comes from plants and fortified foods: lentils, spinach, tofu, fortified cereal, pumpkin seeds. It's absorbed far less efficiently, sometimes under 10 percent, because plant compounds bind to it before your intestine gets the chance. That doesn't make a plant-forward diet a dead end. Vitamin C is the single most useful lever here. Pair your spinach salad with a citrus vinaigrette instead of a creamy dressing — that one swap does more for your iron levels than most supplements ever will.
The blockers are just as concrete. Calcium competes with iron for the same absorption pathway, so a glass of milk or a calcium supplement taken alongside an iron-rich meal can cut absorption significantly; separate the two by at least two hours. Tannins in coffee and black tea do something similar, binding to iron in the gut before it can cross into your bloodstream, which is why a strong coffee with breakfast is one of the more common — and least suspected — reasons an otherwise iron-rich diet still isn't working. Skip the coffee until an hour after an iron-heavy meal, and if you need the caffeine fix earlier, have it black with lunch instead.
Ferritin Is the Test That Actually Tells You Something
Most standard blood panels don't include it.
A basic complete blood count can flag anemia once it's already advanced, but ferritin — the protein that stores iron — drops long before hemoglobin does, which makes it the earlier and more useful marker. Ferritin below 30 ng/mL is widely treated as a sign of depleted iron stores even when hemoglobin still looks normal on paper, and some clinicians flag anything under 50 ng/mL as worth addressing in women dealing with fatigue, hair loss, or heavy periods. If your last checkup only measured hemoglobin, you likely don't actually know your iron status. Women with heavy or prolonged periods, vegetarians and vegans, endurance athletes, recent blood donors, and anyone pregnant or postpartum should ask specifically for a ferritin test — it's a separate line item, not something that gets included automatically in a routine panel.
Supplementing Without Overdoing It
If a blood test confirms low ferritin, ferrous sulfate and ferrous bisglycinate are the two most common supplement forms, typically dosed between 18 and 65 mg of elemental iron depending on how depleted you are and what your doctor recommends. Ferrous bisglycinate tends to cause less stomach upset than ferrous sulfate, which matters, because constipation and nausea are the main reasons women quit iron supplements within the first two weeks. Taking iron every other day rather than daily can actually improve absorption for some people, since the gut's iron-regulating hormone, hepcidin, spikes for roughly 24 hours after a dose and blocks further uptake — a finding that surprised researchers who originally expected daily dosing to work better.
Here's the part that gets skipped in most "just take iron" advice: not everyone should. Hemochromatosis, a genetic condition that causes the body to absorb too much iron, affects a real slice of the population — largely people of Northern European descent — and taking iron supplements without knowing your ferritin and family history can push levels dangerously high over time. Iron overload isn't rare enough to ignore, and it does real damage to the liver and heart. Test first, supplement second, and talk to a doctor before starting anything if hemochromatosis, unexplained joint pain, or liver issues run in your family.
How Much You Actually Need, by Life Stage
The RDA for iron isn't one number — it shifts sharply across a woman's life. Women ages 19 to 50 need 18 mg per day because of ongoing menstrual losses. Pregnant women need 27 mg per day to support the expanded blood volume and fetal development. Women over 50, once menstruation stops, need only 8 mg per day, the same as adult men, because the monthly loss that drove the higher requirement is gone. That drop matters: postmenopausal women who keep taking a high-dose iron supplement out of habit, without ever retesting ferritin, are supplementing for a deficit they no longer have.
Food First, Then a Blood Test
Before reaching for a bottle, look at your plate for two weeks: red meat or dark poultry twice a week, lentils or spinach paired with something acidic, and coffee moved away from mealtimes. If the fatigue, brain fog, or hair loss are still there after that, the answer isn't more willpower. It's a ferritin test and a conversation with your doctor about the actual number on it — not just whether the lab printed the word "normal" next to it.
